Leadership Diagnostic
Find the Real Leadership Problem Before You Invest in the Wrong Solution.
When performance starts slipping, communication breaks down, accountability becomes inconsistent, or every decision keeps moving upward, organizations often respond with more meetings, more policies, or more training.
But if the underlying problem has not been identified, those solutions rarely stick.
The VitalPath Leadership Diagnostic is designed to uncover what is actually happening beneath the surface.
We examine the leadership patterns, structural issues, communication gaps, accountability breakdowns, and decision-making problems that may be limiting performance.
Then we give you a clear picture of what needs to change.

You Know Something Is Off. You May Not Know Why.
Leadership problems rarely arrive with a convenient label.
Instead, they show up as symptoms:
Decisions take too long.
Leaders repeatedly escalate issues they should be able to resolve.
Departments operate in silos.
Managers avoid difficult conversations.
Expectations vary depending on who is communicating them.
Employees are unclear about ownership.
Accountability is inconsistent.
High performers become frustrated or disengaged.
Executives spend too much time solving operational problems.
Meetings generate discussion but little follow-through.
The organization keeps revisiting the same problems.
Growth has created complexity the leadership structure has not caught up with.
Any one of these may look like a people problem.
Often, it is not.
It may be a clarity problem.
A decision-rights problem.
A leadership capability problem.
A structural problem.
An accountability problem.
Or several of them working together.
The Leadership Diagnostic helps separate the symptoms from the root cause.
What Is a Leadership Diagnostic?
The Leadership Diagnostic is a focused organizational assessment designed to identify the leadership issues that are interfering with execution, team effectiveness, and organizational performance.
It is not an employee satisfaction survey.
It is not a personality assessment.
And it is not a generic leadership evaluation.
VitalPath looks at how leadership actually functions inside the organization.
We examine areas such as:
Decision-making
Role clarity
Accountability
Leadership alignment
Communication
Management effectiveness
Escalation patterns
Cross-functional collaboration
Organizational structure
Employee voice
Leadership bottlenecks
Follow-through
Ownership
Executive dependence
The goal is simple:
Identify what is getting in the way so leadership can address the right problem.
Who Is This For?
The Leadership Diagnostic is appropriate for organizations experiencing leadership friction, execution problems, or organizational growing pains.
It may be a good fit if:
Your executive team knows something is not working but cannot agree on the cause.
You have invested in leadership training without seeing meaningful behavioral change.
Managers are technically capable but struggling to lead.
Too many decisions depend on the CEO or senior executive team.
Accountability varies across departments.
Good employees are disengaging or leaving.
Growth has exposed weaknesses in your current leadership structure.
Departments are blaming one another for recurring problems.
Leaders spend more time reacting than leading.
The organization needs an objective outside perspective.
You do not have to know exactly what is wrong before contacting VitalPath.
That is the purpose of the diagnostic.
What We Evaluate
Every organization is different, so the diagnostic is tailored to the situation.
Depending on the scope of the engagement, we may evaluate:
Leadership Alignment
Do senior leaders understand the organization’s priorities in the same way?
Are they reinforcing the same expectations?
Are competing priorities creating confusion below them?
Decision Rights
Who has authority to make which decisions?
Where are decisions becoming unnecessarily escalated?
Are managers empowered to lead, or are they waiting for permission?
Accountability
Are responsibilities clearly owned?
Do leaders follow through consistently?
Are performance expectations enforced equally?
Role Clarity
Do leaders understand where their responsibilities begin and end?
Are there overlapping responsibilities, gaps, or conflicting expectations?
Communication
Is critical information reaching the right people?
Are difficult conversations being avoided?
Do employees understand why decisions are being made?
Leadership Capability
Do managers have the practical skills required to lead people effectively?
Are high-performing individual contributors struggling after promotion into management?
Organizational Structure
Does the current structure support the way the organization actually operates?
Has the company outgrown leadership roles or reporting relationships that once worked?
Leadership Bottlenecks
Is too much authority concentrated at the top?
Are executives solving problems that should be owned elsewhere?
Is leadership dependency preventing the organization from scaling?
Employee Voice
Do employees believe they can raise concerns, challenge assumptions, and offer ideas?
Or have people learned that speaking up is not worth the effort?
How the Diagnostic Works
Step 1: Executive Discovery
We begin with the executive sponsor to understand the concerns that prompted the engagement.
This conversation helps establish:
What the organization is experiencing
Where leadership believes the problems are occurring
What has already been attempted
What outcomes leadership wants to achieve
Step 2: Organizational Assessment
Depending on the size and scope of the engagement, this may include confidential interviews with selected leaders, managers, or key employees.
We explore recurring patterns rather than isolated complaints.
The objective is to understand how the organization functions from multiple perspectives.
Step 3: Pattern Analysis
VitalPath evaluates the information collected and identifies the leadership patterns most likely contributing to the organization’s challenges.
We distinguish between:
Symptoms
and
Root causes.
That distinction is critical.
A communication problem may actually be an accountability problem.
A performance problem may actually be unclear ownership.
A management problem may actually be a structural problem.
A turnover problem may actually be a leadership behavior problem.
Step 4: Executive Findings & Debrief
Leadership receives a clear, direct assessment of what VitalPath identified.
The debrief focuses on:
Primary leadership concerns
Recurring organizational patterns
Likely root causes
Risks if the issues remain unresolved
Priority areas for intervention
Recommended next steps
No 70-page report designed to sit on a shelf.
The goal is clarity and action.
What You Receive
At the conclusion of the Leadership Diagnostic, your organization receives:
Executive findings summary
Identification of key leadership breakdowns
Root-cause analysis
Priority recommendations
Immediate action opportunities
Longer-term recommendations where appropriate
Executive debrief session
Recommended path forward
Depending on the scope, additional documentation or leadership-development recommendations may be included.
What Happens After the Diagnostic?
Sometimes the organization only needs clarity.
Leadership receives the findings, implements the recommendations internally, and the engagement ends there.
In other situations, the diagnostic identifies problems that require additional support.
VitalPath may recommend:
For organizations that need structured intervention to change leadership behaviors, clarify responsibilities, strengthen accountability, or improve execution.
For executive teams that need ongoing leadership guidance, organizational problem solving, and an experienced outside perspective.
Targeted Leadership Development
When the underlying issue is a specific management or leadership capability gap.
The diagnostic does not obligate your organization to purchase another service.
Its purpose is to determine what the organization actually needs.
What Makes the VitalPath Approach Different?
We Do Not Begin With a Training Program.
Training is valuable when lack of knowledge is the problem.
But many organizational problems are not training problems.
Teaching managers another leadership framework will not solve unclear authority.
A communication workshop will not fix inconsistent accountability.
Another team-building exercise will not correct structural dysfunction.
And motivational leadership training will not solve an organization where employees have learned that speaking up carries consequences.
That is why VitalPath begins with diagnosis.
Understand the problem first. Then choose the intervention.
Built From an Operator’s Perspective
VitalPath approaches leadership differently because our work is grounded in real executive operating experience.
We understand that leadership challenges do not exist in isolation.
They affect:
Operations
Employee retention
Productivity
Customer experience
Organizational growth
Culture
Financial performance
Strategic execution
Our focus is not simply on whether someone is a “good leader.”
The more important question is:
Is the leadership system helping the organization perform?
Across Industries. Wherever People Lead People.
Leadership breakdowns are not unique to one sector.
VitalPath works across industries with organizations experiencing leadership, accountability, communication, execution, or organizational-growth challenges.
Our executive experience in healthcare provides particular depth within complex, highly regulated environments, but the principles behind effective leadership apply broadly.
Wherever people must make decisions, work across teams, manage accountability, and execute strategy, leadership matters.
Frequently Asked Questions
Is this an employee survey?
No.
The Leadership Diagnostic is a focused assessment of leadership and organizational patterns. Depending on the engagement, selected leaders or employees may be interviewed, but the purpose is not to measure general employee satisfaction.
Are interviews confidential?
When confidential interviews are part of the engagement, individual comments are not attributed unless there is an agreed-upon reason to do so.
The goal is to identify organizational patterns, not create a list of who said what.
How long does the diagnostic take?
Most diagnostics can be completed within approximately two to four weeks, depending on organizational size, availability, and scope.
Do you work only with healthcare organizations?
No.
VitalPath works across industries.
Our founder’s healthcare executive experience gives VitalPath particular insight into complex organizational environments, but the Leadership Diagnostic is designed for organizations in any industry.
Do we have to purchase another service afterward?
No.
The Leadership Diagnostic is a standalone engagement.
If additional support would be beneficial, VitalPath will explain the options, but there is no requirement to continue.
What if we already know what the problem is?
You may.
The diagnostic can confirm whether the issue leadership sees is actually the root cause—or reveal additional factors that need to be addressed before investing in a solution.
Stop Treating Symptoms.
If your organization keeps solving the same leadership problems over and over, the issue may not be effort.
It may be that the real problem has never been clearly identified.
The Leadership Diagnostic gives your executive team an objective view of what is happening, why it is happening, and where to focus first.
Find the root cause. Create clarity. Move forward.
